File it yourself through the Department of State
You can file the Certificate of Organization yourself through the Department of State's Business Filing Services for the $125 state fee, with nothing added.
It is the same filing a service submits. The form asks for the name, the registered office or a commercial registered office provider, the organizers and an effective date, and it goes in with a docketing statement. Online filings still get human review, and same-day processing is an extra $100 if you need it.
Doing it yourself costs an hour of reading. For a founder already working through the Department of Health's packet, that hour is often the easiest one of the process.

Check the yearly filings before you pick a service
Whoever forms the LLC, Pennsylvania's $7 annual report is due by September 30 every year, and missing it will lead to dissolution starting with the 2027 reports.
