The Department of State annual report and its deadline
Pennsylvania LLCs file a $7 annual report with the Department of State by September 30 each year; corporations file theirs by June 30.
The requirement is new. It began on January 1, 2025, which is why many owners of agencies formed before then have never heard of it. Filed online, the report is approved automatically within minutes, so there is little reason to leave it to the last week.
The report confirms the entity's basic details. Treat the September 30 date as fixed in your calendar, next to your license renewal, because the two protect each other.

What happens when the annual report is missed
From the 2027 reports on, a Pennsylvania entity that skips its annual report faces administrative dissolution six months after the due date, and its name opens up for others.
For a licensed agency that is a real risk. Your Department of Health license names the entity, and a dissolved entity is a poor thing to have printed on a license. The name becoming available is worse: someone else could register the brand clients know you by.
Domestic entities can reinstate with no time limit, paying a reinstatement fee of $35 to $40 plus $15 for each report they missed. That is cheap in dollars but it is disruption you do not need during a survey or a payer contract review.

The license renewal is a yearly filing too
Treat your Department of Health license renewal as part of the annual filing calendar: it is due at least 60 days before expiry, with a $100 fee per location.
Owners tend to file the renewal under operations and the annual report under paperwork, and so they end up owned by different people in the office. Put them in one calendar. A lapse in either can end your ability to serve clients, one by losing the license and the other by dissolving the entity holding it.

Waiver providers requalify at least every two years
Agencies serving Medicaid waiver clients must update and resubmit their provider qualifications at least every two years, and send a copy of each renewed license.
That is a separate cycle from the health license and the annual report, and it runs through the Department of Human Services' long-term living program. It is where insurance certificates, the license and fiscal documents get looked at together.
